This is to affirm that, I authorize Tv Service and its Partners to charge the above Debit/Credit Card against the purchase of their services.

Furthermore; I understand that:

• I have approved this transaction and will hold Tv Service. innocuous with respect to these instructions. I don’t have any concern if my card is charged in multiple splits as long as the total amount remains equal as authorized above.

• I have been well-versed about the transaction split and I will not claim any transaction back made by Tv Service. and /or its partners for the above authorized amount.
